Total Win Bar
Every spin, a bar beside the grid accumulates two things: wins banked from removed Wrapped Candy at the top, and spin multipliers from removed Wrapped Chocolate at the bottom. At the end of the spin, the total multiplier is applied to the accumulated win. It’s the scoreboard the whole game feeds.
The wrapped symbols land like any other symbol, one or more per spin, and they hold their values until something removes them:

The Molar Reel
A reel of teeth sits on the right side of the board in both the base game and the bonus, and on any spin it can reveal one of three modifiers. Only one fires per spin:
- Sticky Fingers – removes all visible Wrapped Candy and Wrapped Chocolate, banking their values and causing a cascade. No further wrapped symbols can land during that spin’s cascades.
- Fairy Flick – removes all high-pay, low-pay, Wrapped Candy and Wrapped Chocolate symbols from a randomly selected section of the grid, causing a cascade. Scatter, Wild and Rotten Wild survive it.
- Sugar Swirl – converts every visible low-paying symbol into the highest-paying low symbol on the grid. Needs at least two different low symbols showing.
Only one of the three can activate on a given spin, so there’s no stacking Sticky Fingers into Sugar Swirl. It’s a single bite of the apple each time, and which bite you get is down to the tooth reel rather than anything on the grid itself.

Rotten Wild
The Rotten Wild is earned rather than landed: after three winning combinations in a single spin, counted across cascades, it drops onto the grid with the next cascade. It substitutes for all high and low symbols, and every winning combination it appears in gets multiplied by a random value from the following:
- 2x, 5x, 10x, 15x, 20x, 30x or 40x
Once it has taken part in all visible wins, it’s removed. The standard Wild is simpler: it substitutes for high and low symbols too, and only one can be visible at a time. It clears once it has done its job, and neither wild will stand in for a Scatter, a Wrapped Candy or a Wrapped Chocolate.
Toothrot Tilly’s Cavity Clash Bonus
Land three Scatter symbols at the same time in the base game and the Cavity Clash bonus begins with 10 free spins. All the base game mechanics stay in play, but Scatters themselves can’t appear during the round, so there’s no retrigger.
The centrepiece is the Beam Battle. A beam starts in a neutral middle position and can travel up to four spaces towards Tilly or the Tooth Fairy. Golden-framed Tilly or Tooth Fairy symbols that land push the beam one space towards the other side.

The two sides aren’t the same fight. Tilly’s end of the beam stacks wrapped symbols and strips the cheap ones away; the Tooth Fairy’s end guarantees the features themselves, including the multiplier-carrying Rotten Wild, which never arrives in the base game without three chained wins first. Which side you back is a genuine choice, and the round can swing either way if both golden symbols land together.

Playing Toothrot Tilly is mostly waiting for the tooth to do its work. The valuable symbols arrive wrapped, and they sit there looking pretty until a Molar Reel modifier unwraps them, so the rhythm of a session is long stretches of ordinary cascades punctuated by a sudden payday. The reason to choose it over plainer scatter-pays games is the bonus: watching Tilly and the Tooth Fairy shove that beam back and forth, with every golden symbol genuinely swinging what the next spin pays, is proper drama.
That Beam Battle is the best thing here. The four-space guarantees are big enough to fight for, and the two sides hand out different prizes, so the tug-of-war is a real contest rather than decoration. The weakness is everything outside it – the base game keeps its best tools locked away, and a tooth reel you can’t influence decides when they open.
It’s a good fit for anyone who likes a bonus round with a bit of narrative and doesn’t need the base game to deliver constantly. Players after non-stop action from spin one will find the sweet shop quiet between visits from the tooth fairy. For the patient ones, the fight over the jars is worth the wait.
